Pernicious anemia case in Uganda
Rural health challenges in Western Uganda: pernicious anemia masquerading as recurrent lower limb weakness-a case report.

We focused on a case of pernicious anemia in a 51-year-old Ugandan man who presented with recurring lower limb weakness. After initial treatment for vitamin B12 deficiency years ago and further testing, we confirmed his diagnosis of pernicious anemia marked by low cobalamin levels. Thanks to timely parenteral methylcobalamin therapy, he showed improvement. We explored three cases of biotinidase deficiency (BTD) that were misdiagnosed as neuromyelitis optica spectrum disorder (NMOSD) due to similar symptoms and neuroimaging results. Initially, two patients responded to immune therapy but later relapsed, while one showed no improvement. Interestingly, after timely diagnosis and biotin supplementation, two patients exhibited remarkable recoveries in both motor function and vision. This highlights the critical need for accurate diagnosis, as early treatment of BTD can lead to significant improvements.

We aimed to see if vitamin B levels could signal the severity of Guillain-Barré syndrome (GBS) and its potential for complications like respiratory failure. Our study involved 50 GBS patients, where we examined their vitamin B and creatine kinase levels alongside their motor abilities.

Interestingly, we found that 44% of patients had low vitamin B levels, which linked strongly to GBS severity. However, while vitamin B showed predictive value, the creatine kinase levels did not relate to GBS severity.
